What This Study Examined

aDG conditions are caused by problems with a protein called alpha-dystroglycan, which normally acts like a molecular anchor, connecting muscle fibers (and cells in the brain and eyes) to their surrounding support structure. This connection depends on a complex sugar coating (called matriglycan) that gets built onto the protein through a multi-step assembly line involving many different genes.

When any gene in that assembly line is faulty, the sugar coating doesn't form properly, and the anchor doesn't work — leading to muscle, brain, and eye problems whose severity depends on how much function is lost.

This review examined the full range of aDG conditions, from the most severe (affecting the brain before birth) to the mildest (adult-onset muscle weakness), and summarized where new treatments stand.

Key Findings

One assembly line, many conditions: All forms of aDG stem from disruptions to the same sugar-building pathway, just at different steps — which is why severity varies so widely even though the underlying problem is related.
Severity spans a wide spectrum: From Walker-Warburg syndrome (the most severe form, affecting brain and eye development before birth) to muscle-eye-brain disease, to Fukuyama congenital muscular dystrophy, to milder limb-girdle forms that may not appear until adulthood.
FKRP-related disease (LGMDR9) is the most common form — and the furthest along in treatment development.
Gene therapy is in clinical trials: Two AAV gene therapies delivering a working copy of the FKRP gene (ATA-100 and AB-1003) are in early-phase trials. Preliminary results from ATA-100 showed an 86% average drop in a muscle-damage marker (creatine kinase) and a 30% increase in the sugar coating on muscle cells just 3 months after a single treatment.
An oral drug shows promise too: A ribitol-based compound (BBP-418) that helps rebuild the sugar coating showed sustained improvements over 15 months in an early trial — and a large Phase 3 trial (FORTIFY) recently announced positive topline results, including better muscle enzyme levels, walking speed, and breathing function.
A surprising discovery about disease severity: Researchers found that in some cases, a shorter, partial version of the sugar coating can still form even when a gene (POMK) is completely missing — explaining why some people with "complete" gene loss have milder disease than expected. This finding is opening new therapeutic ideas about boosting this partial repair mechanism.

Why This Matters

For decades, aDG has had no treatment beyond managing symptoms. This review shows the field has now moved from understanding why these conditions happen to actively testing therapies that address the root cause — either by delivering a working gene (gene therapy) or by directly supplying the raw materials needed to rebuild the missing sugar coating (ribitol therapy).

The FORTIFY trial's positive topline results are a particularly encouraging signal, since a positive Phase 3 result is often the last major step before a therapy can be reviewed for approval. While FKRP-related disease is currently the furthest along, the shared biology across aDG genes means lessons learned here — including new tools to measure disease progression — could help pave the way for treating other aDG subtypes in the future.
